HOUSING AUTHORITY OF THE COUNTY OF DAUPHIN
MISSION STATEMENT:
The Wellness Center is a nurse-managed practice operated in partnership between the Messiah College Department of Nursing and the Housing Authority of the County of Dauphin to supplement existing primary care services in the community. Emphasis is placed on health promotion, disease and injury prevention, risk reduction, and chronic illness monitoring. Services are limited to residents living in Housing Authority apartment complexes.
GOALS:
To improve the health of persons living in Housing Authority buildings
To offer health promotion and health education programs and services
To provide direct access to limited primary care services
To provide a community-based clinical experience for nursing students
SERVICES:
Health and safety assessments and screenings
Health education and risk reduction counseling and programs
Chronic illness monitoring
Medication assessment, education, and management
Blood pressure and weight monitoring
Nutrition assessment and education
Collaboration with Michael Weisberg, Senior Citizen Service Coordinator, physicians; social workers; nutritionists; Area Agency on Aging; home health and medical equipment companies; and other community services and resources
